Eligibility:
Applicant must:
Be a member in good standing
of St. Mark Evangelical Lutheran Church located in Worth, Illinois.
Submit a current copy of
a transcript from a high school, college, or other institution of higher learning. (A computer copy is acceptable.)
Scan or attach a current picture.
Application:
A previous recipient of an award may apply again.
Before submitting this application,
be sure that you have complied with all the requirements printed above.
Due date: By 3:00pm (Central
Time) Friay, May 22, 2009 (Church office hours are: Tuesday - Friday 9:00am - 3:00pm (CST)
Your
completed application must be HAND-DEIVERED to the church office or E-mailed by the due date.
Please note that there are no exceptions to the above specifications. LATE APPLICATIONS
WILL NOT BE CONSIDERED.
